issue) under my sponsorship to work at my residence for initial contract period
of two years keeping the renewal of contract for further period open between
the housemaid and myself. An Employment Contract duly verified and attested by
the Ministry of Justice or Ministry of Labour of the Government of State of
Qatar is enclosed herewith for attestation by the Embassy of Nepal, Doha-Qatar.
Also enclosed is Bank Guarantee worth Qr. 4,000/- (Four Thousand) only for the
provision of emergency and non-cooperation from my side.
I shall be abide by the following terms and conditions
as stipulated by the Embassy of Nepal, Doha-Qatar for the employment of this
housemaid:
1. I will bring the housemaid to the Embassy within
two months of her arrival thereby completing all the processes i.e. visa
stamping and opening bank account under her name and in every four months
following the first appearance to make the Embassy acquaint with her status and
welfare.
2. The Embassy can periodically speak to the maid at
telephone no. ---------------------of my
residence or
her direct contact no. ---------------------, subject to my consent.
3. I will transfer an amount of Qr. 1,000/-(One
thousand) only to the maid's account against the payment of her monthly salary
and take her to withdraw the amount for the purpose of remitting the same to
her family back in Nepal as per her will.
4. I will provide free visa, free onward (joining)
and return air passage after the completion of contract period, resident
permit, food, safe accommodation, free medical treatment and other daily
personal logistics including the sanitation items during menstruation.
5. I assure the Embassy that I will bring my
housemaid using the Tribhuvan International Airport in Kathmandu, Nepal.
6. I have attached herewith the approval issued by
the Ministry of Interior of the Government of Qatar under my name to bring
housemaid from Nepal for the kind reference of the Embassy subject to
verification from the Mission.
7. The age of my proposed housemaid is not less than
30 years and she is unmarried / married with her husband and her............

including the children for the maid to deliver her service and will arrange
extra housemaids if the number exceeds to that as declared in this guarantee
and undertaking.
9. I take the full guarantee to provide a decent
bachelor accommodation, air conditioner and proper ventilator to my housemaid
once she joins in service.
10. I will provide the first aid box in her
accommodation and in my kitchen for use of the housemaid in case of emergency.
11. I take the responsibility to orient the housemaid
for a minimum period of seven days in her responsibility i.e. using utensils,
kitchenware and appliances etc. immediately after her joining the service.
12. I will inform the Embassy of the housemaid's
departure and arrival from any third country along with my family on holiday or
tour or
voyage.
I will not supply the maid to any third party within and outside Qatar. She
will be employed solely for the service of my family.
13. I will provide the maid a 21 day paid leave after
completion of one year and if she wants to go for vacation after successful
completion of two years of contract period, I will provide 42 day salary as
leave pay along with an up and down air passage. Even if she goes for
cancellation after the completion of contract, I will provide her the leave pay
and a return air passage to Nepal.
14. I will obtain (Life Insurance worth QR.
200.000/00 in words "Two Hundred Thousands Qatari Riyals" to the
designated housemaid during the period of contract before or after her entry to
the country. If I didn't do so, I will be obliged to pay compensation of
QR.200.000, in case of death of the housemaid.
I am aware that the Embassy can ask me to send the
maid back to Nepal at my expenses from the bank guarantee kept as deposit at
the Embassy prior to bringing the housemaid.
I will remain eligible to claim
the deposit back from the Embassy only after fairly paying all the dues and
satisfying the maid on the date of her departure from Doha.
